Bug description: Whenever I place a monitor, basic or advanced, it works perfectly fine, but when I leave the server and join again, upon chunk loading the game completely crashes, and Java creates an crash report regarding my video card.

Given that it's the card driver actually crashing, I'd start out by updating that.
Oh, and don't go and manually download and install the "latest ATI driver", that being the easiest one to find. Odds are it won't support your card and will leave you on your current version. Ensure you get the latest driver download for your card.

Thanks for the quick reply, but the driver is not the issue, since the driver is the latest one available for my video card. I have a relatively old model.

Again, I don't believe you're running the latest driver. I stress this because with this manufacturer, it's easy enough to get the wrong file and have it tell you you're up to date when you're not.
Also note that it is the driver that is crashing. If you're truly up to date, then there's nothing for it but to report the issue to the manufacturer of that driver.
What I'd recommend for now is listing out the model of the card and the version of the driver you're currently using.

Ok, so a Radeon X1550 Series card on version 8.593.100.0. It doesn't look like there's a Windows 8 specific driver available, but assuming Windows 7'll work, give this a shot:

Yeah, it's already up to date. The suite is version 10.2, but the driver it installs is
Radeon™ display driver 8.593, which is the current driver I'm running. Any other ideas?
Also, I shall mention that this error
ONLY occurs on multiplayer and it
ONLY occurs upon reloading the chunk that the monitor is contained in. When placing it down once in-game, I have no problem using it and no errors/crashes occur.

Hate to say it, but you might be stuck. I ran into much the same thing - random crashes shortly after login to a SMP server with the ATI driver file to blame; lucky for me, my card supports a later driver version then yours (… and not by much, at that). In my case the trigger I ran into was paintings. I'd be highly surprised if there's only two.
As there won't be any more updates released for your card (if you were to bug AMD they'd likely suggest you buy a new one), that leaves the rather hit-and-miss tactic of attempting to delete your current driver installation and replacing it with older ones in the hopes of hitting on a version before the bug was introduced.
There is one other "simple" option, though it's a bit of a long shot, so I wouldn't get your hopes up - in MineCraft's video settings is a toggle for advanced OpenGL. Maybe try flipping it and see what happens.
