Help with this program

Hey all.
Heres another one of my questions! :)/>

So i would like to have a program (Lets call it bookings) and i will have it at my front desk at reception in my server.
And anyone who wants to book a meeting with me clicks the computer and it will ask them to enter there name and desired time.
Maybe i could get it to look like this.

[left]Please enter your name, in game time you would like an appointment[/left]

[left]Name/Time: (They would enter here)[/left]

[left](and after they enter there info it goes to a screen that reads "Your appointment is now booked!")[/left]

[left]then it takes you back to the appointment booking sheet where another person can enter an appointment.[/left]

[left](Notes) I need a way to be able to see all the appointments they have booked in the same program or another program either is fine.[/left]

Added reason for fun :P/>, splitting may not be 100% working.
Change the admin table to have your user details for login to check the bookings list.

People will be able to bypass it using a disk drive, so more security is up to you.
